SCL


Services provided through CCI’s Supported Community Living (SCL) program support and promote the individuals’ ability to live, work, and actively participate in the community. Participants in the SCL program set their own goals and objectives, according to individual need and choice of services, to help them transition from residential care, and reach and maintain the highest possible level of independence.

SCL ResidentsWho is eligible?
SCL services are available to individuals 18 years of age and older who have a diagnosis of chronic mental illness. Case managers, family members, mental health care professionals, or the individual may make a referral to CCI’s SCL program.

Where is the program offered?
Hourly and 24-hour services in the individual’s home are available in Clinton, Scott, Marion, Chickasaw and Jones counties with hourly services only offered in Jackson and Howard counties.

What services are provided?
SCL program participants share community living areas in SCL homes, plan and prepare meals together, and engage in leisure activities. Each individual is encouraged to personalize his or her private bedroom by décor, color and theme. Individuals choose services based on the needs identified in their personal plan toward independence. SCL services include:

  • Medication management
  • Mental health symptom monitoring
  • Stress management and socialization skills
  • Budgeting
  • Menu planning and meal preparation

Thank you to the Scott County Housing Council for funding Community Care, Inc.’s first SCL home in Davenport.  Five individuals moved into their new home in June 2010.  CCI will open our second SCL home in Davenport in June 2011 with help from funds provided by the Scott County Housing Council and the Scott County Regional Authority.

CCI gratefully acknowledges the Iowa Northland Regional Housing Council’s investment of $5,500 and First Citizens National Bank Foundation’s $1,500 contribution to assist CCI in the purchase and installation of new windows for our SCL home in New Hampton, Iowa.
