CCI’s Residential Care Facilities (RCF) programs are community-integrated programs offering services to adults with disabilities who require licensed residential treatment services. The home-like setting provides each individual with the support, supervision, training and rehabilitation needed to remain in the least restrictive environment and to prepare for a more independent community living arrangement. Marion County Care Facility

Who We Are
Marion County Care FacilityThe Marion County Care Facility is located in a peaceful rural setting near Knoxville, Iowa. We provide 24-hour supervision and medication assistance to individuals with chronic mental illness and developmental and/or intellectual disabilities. Our on-site clinic is staffed weekly by the Facility Medical Director. On-site psychiatric services are available monthly to individuals, providing a high quality continuum of care. Activity staff and Direct Support Professionals assist residents with independent living skills and a variety of enjoyable activities to fit individual needs.

What We Offer
At the Marion County Care Facility, we strive to meet individuals’ needs in a comfortable and pleasing environment that encourages personal growth and comfort. Our residents enjoy a large activity room and have access to several lounge areas where they can relax together or visit with family and friends. Our recreation department offers a variety of therapeutic community activities for individuals. Residents play “Wii’ games, cards, board games or pool during leisure time and have access to fitness room equipment and an on-site beauty shop. Community meals are served in our dining room. In warm weather months, the private patio is available for barbeques and outdoor gatherings. A small apartment setting assists individuals with independent living skills.

Heritage Residence

Who We Are
Heritage ResidenceThe Heritage Residence, formerly known as the County Home, was established in 1907 and used as a working farm in rural Chickasaw County near the county seat of New Hampton. The current facility was constructed behind the site of the original building. Adults 18 years of age and older who have been diagnosed with chronic mental illness, brain injury, intellectual disability, physical disability and/or developmental disability are provided 24-hour care at Heritage Residence.

What We Offer
At Heritage Residence, individuals receive round-the-clock personal assistance and supervision from highly trained, professional staff. Health screening, social and independent living skills training, recreational skill development, and behavioral therapy help individuals prepare to make the transition to community living. Our spacious dining room and multiple leisure activity areas promote and encourage socialization. Our aviary in a central gathering space adds a unique amenity for residents to enjoy.  Thank you to the Chickasaw County Community Foundation for the Spring 2011 grant award of $2,500 toward the purchase of recreational therapy equipment and furnishings at Heritage Residence.

Fairview Care Facility

Who We Are
Fairview Care Facility opened to individuals with disabilities on November 15, 2010. Community Care, Inc.’s Board of Directors agreed to purchase the former Jones County Regional Medical Center in Anamosa, Iowa and chose “Fairview” as the name for the new facility in honor of the Jones County village of Fairview that was settled in 1841.  Individuals diagnosed with chronic mental illness are referred to CCI for 24-hour residential care at the 36-bed facility, located at 104 Broadway. Thank you to the Anamosa Community Foundation for awarding $200 in grant fund support toward the purchase of the main entrance sign at Fairview Care Facility and to the Jones County Endowment Fund for $2,000 in grant funds to purchase appliances for Fairview Living Skills Center.

What We Offer
Medication management and treatment specific to diagnosis, delicious meals and comfortable housing, recreation programs and pre-vocational programs designed to assist individuals toward their goal of community living and reintegration.
