ICF/ID
CCI’s Intermediate Care Facilities for the Intellectually Disabled (ICF/ID) program integrates programs and services through local community resources to intellectually and developmentally disabled and brain injured adults needing intense training and/or medical monitoring provided in a home-like environment.
Who is eligible?
Persons 18 years of age or older with a primary diagnosis of intellectual disability needing a high degree of structure and support may be referred for ICF/ID placement by case managers, income maintenance workers, and Iowa Medicare Enterprise.
Where is the program offered?
CCI first offered ICF/MR (Intermediate Care Facilities for Persons with Mental Retardation) services in 1989 on the third floor of the Charlotte, Iowa facility. Four ICF/ID homes were built in DeWitt, Iowa in 2004, offering individuals specialized care in a community setting. Each residence provides shared areas and private rooms for 6-7 individuals. Training programs are tailored to develop maximum independence. Family members and individuals are part of the planning team and are encouraged to participate in all aspects of individual program development.
What services are provided?
CCI staff trained in First Aid, CPR, Universal Precautions and Advanced Mandt training deliver 24-hour care and assistance with daily living activities that include:
- Medical monitoring and medication management
- Grooming and personal hygiene
- Occupational and physical therapy
- Behavior modification
- Speech therapy
- Social and vocational services
- Recreation therapy and activities
- Transportation in lift-equipped vehicles
