Home Health Care (HHC) assists individuals to achieve the highest level of independence in the least restrictive environment. HHC promotes and maintains individual health and well being by coordinating medical treatment with their physicians to ensure quality and continuity of care and helps the individuals with personal daily living activities in their own homes.

Who is eligible?
Home Health Care services are available to individuals of all ages diagnosed with developmental disabilities and mental illness.

Where is the program offered?
Skilled nursing and personal care services are available in Clinton, Scott, Jackson and Jones counties. Funding sources for services may include Medicaid, private insurance, county funds and private pay.

What services are provided?
Registered Nurses and Home Health Care Aides work as a team to provide quality health care and assistance with daily living activities. All services are provided according to physician’s orders and are available Monday through Friday with 24-hour on-call nursing services provided 7 days a week. Post-operative care and ongoing care for traumatic brain injuries and other chronic health conditions – such as cardiac care, diabetes, and epilepsy – is provided by CCI’s home health care professionals.

HHC Skilled Nursing services include:

  • Assessing individual’s health condition
  • Providing medical treatment as needed
  • Educating individuals on medications and overall health

Additional services provided by HHC include:

  • Respite Care Services for the individual’s caregiver
  • Interim Medical Monitoring and Treatment specialized daycare services for individuals age 20 and under
  • In Home Care Programming
