Home and Community Based Services (HCBS) offer individuals with disabilities choice-driven support to maintain an optimum level of independence in their own homes and communities. Support received though CCI’s HCBS program empowers individuals to develop and work toward personal goals, learn effective communication skills, and participate in the community.

HCBS ResidentsWho is eligible?
CCI’s HCBS program is available to individuals of all ages who are eligible for services under the Intellectual Disabilities (ID) Medicaid Waiver and the Brain Injury (BI) Waiver.

Where is the program offered?
CCI offers 24-hour and hourly HCBS services in Scott, Clinton, Jackson, Jones, and Chickasaw counties and hourly services only in Cedar, Linn and Howard counties. Services are scheduled according to the individual’s needs, whether the individual lives alone, with family members or a caregiver, or with roommates in an HCBS home owned or managed by CCI. HCBS Direct Support Professionals employed by CCI receive training and certification in CPR, First Aid and MANDT.

What services are provided?
Services under the Medicaid Intellectual Disability (ID) Waiver include:

  • Supported Community Living (SCL) assisting the individual with independent living skills development
  • Respite Care Services giving temporary relief to the individual’s caregiver
  • Home and Vehicle Modifications helping individuals achieve independence through physical modifications to their home or vehicle
  • The Personal Emergency Response System ensuring 24-hour access to emergency assistance
  • Home Health Aides assisting in daily living activities under physician’s order
  • Interim Medical Monitoring and Treatment providing specially trained caregivers for persons age 20 and under to enhance the individual’s social, emotional, intellectual, and physical development
  • Supported Employment providing job training and job placement services through CCI Industries

All of the above services, and Consumer Directed Attendance Care (CADC) to help individuals with daily self-care, are offered under the Medicaid Brain Injury Waiver.  Thank you, Theisen’s of DeWitt, for awarding a $2,000 “More for Your Community” grant to CCI’s Home and Community Based Services program in 2010 and to the Scott County Housing Council for awarding a $25,000 grant to purchase and renovate an HCBS home in Scott County in 2011.
